A one-page appointment brief
- Purpose: the question or decision you want help with.
- Known dates: dates already on documents or confirmed by an organisation.
- Timeline: a few events in order, distinguishing records from recollection.
- Documents: what you have, its date and version, and what is missing.
- Three questions: put the most pressing first.
- Next steps: the agreed task, responsible person and timing.
Keep this brief on your own device or on paper. Do not enter private facts in our search planner. Ask the verified provider how to share any documents securely.
